Documents for New Partners
The following Documents need filling out and returning to Diverse Utility Solutions.
Partner Licensee Agreement

This is the licensee agreement between you and Diverse Utility Solutions Limited.
Self Billing Agreement

This is the document that you need to sign in order to receive commission statements and self billing invoices form Diverse Utility Solutions Limited.
Lead Application Form

This is the document that you need to fill out and sign in order to receive leads for new prospects from Diverse Utility Solutions Limited. (Partners Only)
We also require two form of identification, this can be one of the following - 

List A 
  • A British Passport -  showing that you are a British citizen or a citizen of the United Kingdom and Colonies and have the right of abode in the United Kingdom
  • A passport or national identity card showing that you are a national of the European Economic Area or Switzerland
  • A residence permit, registration or certificate or document certifying or indicating permanent residence issued by the Home Office, Border and Immigration Agency or UK Border Agency to a national of a European Economic Area country or Switzerland
  • A permanent residence card issued by the Home Office, Border and Immigration Agency or UK Border Agency to a family member of a national of a European Economic Area country or Switzerland
  • A passport or other travel document endorsed to show that the holder is exempt from immigration control, is allowed to stay indefinitely in the United Kingdom, has the right of abode in the United Kingdom, or has no time limit on their stay in the United Kingdom
  • A full birth certificate issued in the United Kingdom which includes the name(s) of at least one of the holder’s parents, when produced in combination with an official document giving the person’s permanent National Insurance Number and their name issued by a Government agency or a previous employer
List B 
  • Valid Driver's License.
  • Property Tax Receipt.
  • Posted Mail with name of applicant.
  • Utility Bill.
  • Lease Agreement.
  • Insurance Card.
  • Voter Registration Card.
Should you not be able to produce one item from List A and also one item from List B we will need to hold your application until we are able to have an interview in person. This is to comply with our work ethics and also the Modern Slavery Act 2015.


All these documents will need sending to admin@dus.co.uk once you have completed the signup process.
