A Simple Direct Debit.
It is important that the client fills out this form. You are NOT permitted to fill out the direct debit instruction on behalf of your client.
The Direct Debit instruction must be filled out and signed by the client to make the form valid.
In some cases, the form might need to be signed by someone else. For example a church treasurer, finance manager etc.
It is sometimes worthwhile asking this question in the first meeting to ensure business can be fully completed on the second visit.
