How to fill out the Waste DCF
The first part of the form is very straight forward. Insert the business details, including the business name, contact details and the site address.
We will require the clients email address in order to send the quote to the client.
You must also fill out your ID Number, as this will link this client to you.
Risk Assessment
- This will need to be completed in order to proceed with pricing. You can either perform the risk assessment on-site, over the phone or via email.
The risk assessment comprises of three main questions: site access, potential hazards and risk level overview. This is a rough overview to hep pricing. If you are unsure about any component please leave blank.
Site Access
- It is important for us to know any access issues so we are able to safely judge the correct container size and let the supplier know what potential issues there may be in getting the waste collected.
Potential Hazards
- Tick as appropriate and note other information you think would be useful for us to know.
Risk Level
- What risk level is there in emptying the bin? Low, Medium or High.

The form asks for the quantity of each bin, the size and how often it is emptied
The last boxes are the client's signature to agree all of the above information is correct and allows us to find prices with this information.
